EverWell Wound Care, PLLC is required by law to maintain the privacy and security of Protected Health Information (PHI), provide this Notice, and follow its terms.
We will notify affected individuals following a breach of unsecured PHI as required by law.

Treatment: To provide, coordinate, and manage healthcare services.
Payment: To obtain reimbursement from insurers or responsible parties.
Healthcare Operations: For quality improvement, compliance, auditing, credentialing, and administrative purposes.

Other Permitted Disclosures
